Case Coordinator – Transition House

The Transition House Coordinator will support timely access to care across several programs, including MST, IIH, Day Treatment, and PRTF. Reporting to the Director of Access Services, this role helps remove barriers to treatment by assisting with intake coordination, documentation, and scanning needs. It’s a great opportunity for someone who is organized, detail-oriented, and passionate about helping families connect to vital mental health services.
