Case Coordinator – Transition House

The Transition House Coordinator will support timely access to care across several programs, including MST, IIH, Day Treatment, and PRTF. Reporting to the Director of Access Services, this role helps remove barriers to treatment by assisting with intake coordination, documentation, and scanning needs. It’s a great opportunity for someone who is organized, detail-oriented, and passionate about helping families connect to vital mental health services.

Southern Pines, NC
Intensive In-Home Coordinator

The Intensive In-Home Coordinator supports children and families through skill-building, parenting education, crisis response, and care coordination in home, school, and community settings. As part of a 3-person team with a 12-family caseload, the coordinator helps implement treatment plans, provides 24/7 on-call support, and works closely with families and community partners to promote stability and reduce the need for higher levels of care.

Registered Nurse – PRTF

The Registered Nurse (RN) ensures 24/7 coverage at our PRTF, supporting client safety, managing medications and treatments, and coordinating care. RNs monitor interventions, respond to health needs, educate clients and staff, and support the Neuro-sequential Model of Care. Flexibility and teamwork are key, with RNs stepping into direct care roles as needed.
