A Whole Solution for Children’s Mental Health
At Alexander Youth Network, we know how important it is for families to feel supported at every step of their child’s care journey. That’s why we offer Tailored Care Management (TCM) — a free service for eligible North Carolina Medicaid members that focuses on your child’s physical health, mental health, and overall well-being.
Your assigned Care Manager works with you to build a plan centered on your child’s strengths and needs. We help coordinate services, keep providers connected, and ensure your family has the tools to thrive. We check in regularly and offer quarterly in-person visits, with flexibility based on your schedule. You’ll always have your Care Manager’s contact information, and you’re encouraged to reach out anytime for support.
Whether you need help with healthcare, school support, housing, or navigating a crisis, we’re here to support your family every step of the way!
How We Provide Care Management

Coordinated, Personalized Care:
Your Care Manager works with you to build a care plan centered on your child’s strengths and your family’s goals. We coordinate with doctors, therapists, schools, and community providers to ensure everyone is working together and that your child’s needs are met as life changes. -
Support and Advocacy:
We’re here to help you understand your options and connect to the resources that matter — from housing and food support to educational services. Your Care Manager is your partner in making informed decisions and advocating for your child’s well-being. -
Health Monitoring & Transitions:
